How the ReveIQ E/M Level Selector works
Office/outpatient E/M codes (99202-99215) allow selection by either total time on the date of the encounter or level of medical decision making. ED E/M codes (99281-99285) allow only MDM-based selection — time cannot be used for ED visits per AMA rules.
Medical Decision Making (MDM) has three elements: (1) number and complexity of problems addressed, (2) amount and complexity of data reviewed, and (3) risk of complications. Final MDM level = the level met by 2 of the 3 elements (or higher). ReveIQ lets you select each element independently and shows the resulting level.
When both methods apply (office visits), ReveIQ shows both results side by side. You may select the higher-level code if your documentation defensibly supports it. ReveIQ does not make the final selection — your coder or biller does. This tool is decision support.
Code 99211 (nurse visit) and code 99281 are special cases that may not require physician presence and are not typically selected based on MDM or time. They are shown when MDM is minimal and time is below thresholds.
